2024 / 2025 LOVELAND PERFORMING ARTS BOOSTERS ANNUAL MEMBERSHIP

1) In the notes section (if applicable), please list all music organizations that your child(ren) participate in so we can add you to those groups for communication purposes.

2) In the organization field, please designate ONE group from the following to support:

H.S. BAND, H.S. CHOIR, H.S. DRAMA, H.S. STRINGS, M.S. BAND, M.S. CHOIR

Fifty Percent of your annual membership dues go directly to the funds of your designated group, the other fifty percent go to the boosters general fund.
